The story follows Bruce McGivern, an agent sent to investigate an ocean liner overrun by infected creatures after a bioterrorist attack. Alongside Fong Ling, a Chinese agent with her own motivations, Bruce must survive the ship’s claustrophobic corridors while uncovering the intentions behind the outbreak and facing a new threat linked to the use of the virus. The atmosphere of isolation, mystery, and tension is constant, reinforcing the horror and conspiracy tone characteristic of the series.

The gameplay mixes third-person exploration with first-person combat moments, creating its own dynamic within the franchise. The player traverses closed environments collecting items, solving puzzles, and managing ammunition and healing resources, maintaining a focus on survival. In confrontations, the view switches to first-person, allowing for more precise aiming, which brings the experience closer to a shooter game without abandoning the classic elements of Resident Evil.
